I am a musician presently employed by EBE Talent, 1030 N. Deleware Ave. Philadelphia, PA 19125. I hereby authorize and designate the Philadelphia Musicians' Union, Local 77 of the American Federation of Musicians, to represent me for purposes of collective bargaining over wages, hours and other terms and conditions of my employment with the aforementioned employer.
